ประกาศสำนักงานมาตรฐานสินค้าเกษตรและอาหารแห่งชาติ เรื่อง กำหนดแบบคำขอรับใบอนุญาต แบบใบรับคาขอรับใบอนุญาต แบบใบอนุญาตเป็นผู้ผลิต ผู้ส่งออก หรือผู้นาเข้า แบบหนังสือแจ้งการส่งออกหรือนำเข้า และแบบใบรับแจ้งการส่งออก หรือนำเข้าสินค้าเกษตรตามมาตรฐานบังคับ อาศัยอานาจตามความในข้อ 8 ของกฎกระทรวงการขออนุญาตและการอนุญาตเป็นผู้ผลิต ผู้ส่งออก หรือผู้นำเข้าสินค้าเกษตรตามมาตรฐานบังคับ พ.ศ. 2563 ออกตามความในพระราชบัญญัติ มาตรฐานสินค้าเกษตร พ.ศ. 2551 เลขาธิการสานักงานมาตรฐานสินค้าเกษตรและอาหารแห่งชาติ จึงกาหนดแบบคาขอรับใบอนุญาต แบบใบรับคาขอรับใบอนุญาต แบบใบอนุญาตเป็นผู้ผลิต ผู้ส่งออก หรือผู้นาเข้า แบบหนังสือแจ้งการส่งออกหรือนาเข้า และแบบใ บรับแจ้งการส่งออกหรือนาเข้าสินค้าเกษตร ตามมาตรฐานบังคับไว้ ดังต่อไปนี้ ข้อ 1 ให้ยกเลิกประกาศสานักงานมาตรฐานสินค้าเกษตรและอาหารแห่งชาติ เรื่อง กาหนด แบบคำขอรับใบอนุญาต แบบใบรับคำขอรับใบอนุญาต แบบใบอนุญาตเป็นผู้ผลิต ผู้ส่งออก หรือผู้นำเข้า แบบหนังสือแจ้งการส่งออกหรือนำเข้า และแบบใบรับแจ้งการส่งออกหรือนำเข้าสินค้าเกษตร ตามมาตรฐานบังคับ ลงวันที่ 16 กุมภาพันธ์ พ.ศ. 2564 ข้อ 2 ให้ใช้แบบคำขอรับใบอนุญาต แบบใบรับคำขอรับใบอนุญาต แบบใบอนุญาต แบบหนังสือแจ้ง และแบบใบรับแจ้งการส่งออกหรือนำเข้ำสินค้าเกษตรตามมาตรฐานบังคับ ตามที่กาหนดไว้ท้ายประกาศนี้ ดังนี้ (1) แบบคำขอรับใบอนุญาตเป็นผู้ผลิตสินค้าเกษตรตามมาตรฐานบังคับ ตามแบบ มกษ. 1 (2) แบบใบอนุญาตเป็นผู้ผลิตสินค้าเกษตรตามมาตรฐานบังคับ ตามแบบ มกษ. 2 (3) แบบคำขอรับใบอนุญาตเป็นผู้ส่งออกสินค้าเกษตรตามมาตรฐานบังคับ ตามแบบ มกษ. 3 (4) แบบใบอนุญาตเป็นผู้ส่งออกสินค้าเกษตรตามมาตรฐานบังคับ ตามแบบ มกษ. 4 (5) แบบคำขอรับใบอนุญาตเป็นผู้นำเข้าสินค้าเกษตรตามมาตรฐานบังคับ ตามแบบ มกษ. 5 (6) แบบใบอนุญาตเป็นผู้นำเข้าสินค้าเกษตรตามมาตรฐานบังคับ ตามแบบ มกษ. 6 (7) แบบหนังสือแจ้งการส่งออกสินค้าเกษตรตามมาตรฐานบังคับ ตามแบบ มกษ. 7 (8) แบบใบรับแจ้งการส่งออกสินค้าเกษตรตามมาตรฐานบังคับ ตามแบบ มกษ. 7 - 1 (9) แบบหนังสือแจ้งการนาเข้าสินค้าเกษตรตามมาตรฐานบังคับ ตามแบบ มกษ. 8 (10) แบบใบรับแจ้งการนาเข้าสินค้าเกษตรตามมาตรฐานบังคับ ตามแบบ มกษ. 8 - 1 ประกาศ ณ วันที่ 20 เมษายน พ.ศ. 256 6 พิศาล พงศาพิชณ์ เลขาธิการสานักงานมาตรฐานสินค้าเกษตรและอาหารแห่งชาติ ้ หนา 36 ่ เลม 140 ตอนพิเศษ 126 ง ราชกิจจานุเบกษา 31 พฤษภาคม 2566
